Discontinuance "Turn off" of Water Service Request Form

If your services were SHUT OFF due to NON-PAYMENT the after hours on-call technician will not be able to assist you. Please contact the utility department during Monday-Friday, 8:30 a.m. - 4:30 p.m. at 609-567-0700 prompt #7.

All discontinuance of Water service due to vacancy must be submitted in writing.  Also a (Discontinuance "Turn Off" of water service request form) must be completed by the property owner.  All requests require a 48 hour notice during normal business hours only.

Please contact the Department of Municipal Utilities to obtain a copy of the proper paperwork or complete the form below and fax or e-mail back so a work order can be issued.
